Amberwell to Give Physicals at THS/TMS on April 30th

Amberwell will give sports physicals to Troy Middle/High School students that will be attending school next year in Grades 7-12 on Thursday April 30th at 8:30 AM. The physical will cost $30 and can be paid by Cash or Check (check made out to Amberwell).  

A KSHSAA physical form must be filled out and signed by the athlete and parents.

A concussion form must also be signed by the athlete and parent.  

A permission slip signed by the parent to allow Amberwell to perform the physical must also be signed.

Also in August Amberwell along with the athletic trainers will perform Impact Testing. The required form with student/parent signatures is attached.
