Physcials will be offered on Friday, May 17th at Troy High School by Cory Schrick, PA-C. Forms with fee and all signed documents are due to the HS office by Wed., May 15th!
Troy High School student-athletes will be given the opportunity to obtain a sports physical, which will satisfy the requirements for athletic participation for the 2024-2025 school year. The physical will be administered by Cory Schrick, PA-C, employee of The Orthopedic and Sports Medicine Center in St. Joseph, MO. The physicals will be given at the high school on Monday, May 17th. The charge for these physicals will be $30, cash or check (made payable to Cory Schrick).
If you would like to take advantage of this opportunity, we would ask for you to fill out the attached forms (also available in the high school office) and permission slip and send them back no later than Wednesday May 15. Your child will not be given a physical unless the permission slip is signed and returned with payment. If you would rather go through your family physician, physicals for fall participation will be due August 19th!
Please make sure physical forms are signed in all the signature spots by the parent and athlete.

If you choose to have a physical done by your own provider over the summer, you will need to have the KSHSAA Physical Form and the Concussion form completed and signed before first day of practice on August 19th. In the summer, forms will be available on the Troy School Website at www.troyusd.org and the Troy Central Office (Mon. - Thurs. - 8 AM - 4 PM).
